Release Form

The Sparkle Spa LLC requires you as the Parent to sign this waiver release form. As part of the party experience, facial masks and age-appropriate makeup will be applied. This includes, but not limited to eye shadow, and face glitter. All makeup products used are sensitive, hypoallergenic and makeup applicators are brand new and disposable. I acknowledge that any of the services are for entertainment purposes only!

PHOTOGRAPHY RELEASE

I give The Sparkle Spa LLC permission to use my child's image, voice, for pictures, photos, video recordings taken during activities. I agree That The Sparkle Spa LLC has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with The Sparkle Spa LLC Mobile Spa Parties. These uses include, but are not limited to, advertisements, social media and any promotional materials, including the internet. I acknowledge that I will not receive any compensation for the use of such pictures, etc., and hereby release The Sparkle Spa LLC, from any and all claims regarding this matter.

FOOD AND ALLERGY WAIVER

We do offer snack packs as part of the experience. We keep everything nut free! We know that many children have allergies and some of these allergies can be life threatening. Safety is our number one priority! We take many precautions to ensure that all children are safe. The Sparkle Spa LLC will not be held responsible for any allergic reaction or life-threatening incident including death that may occur. If your child has any known food or skin allergies, please let us know and we will make substitutions upon your request.

GENERAL LIABILITY WAIVER

We Strictly provide entertainment! Please be aware that children need to be supervised. We cannot be held responsible for accidental damage caused by your child. The Sparkle Spa LLC assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss of our inventory and/or damage or loss to the party bus, the responsibility lies entirely with the client and or guardian. The Sparkle Spa LLC, is not responsible for allergic reactions and / or any medical injuries from the food, beverages, wearing costumes or any activity taking place before, during or after the party.

I hereby release and agree to hold The Sparkle Spa LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or that may otherwise arise in any way in connection with any services received from The Sparkle Spa LLC. I understand that this release discharges The Sparkle Spa LLC from any liability or claim that I, my family, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from The Sparkle Spa LLC. This liability waiver and release extends to The Sparkle Spa LLC together with all owners, partners, and employees.
